The Audit Log provides a tamper-proof record of all staff actions within the practice system. This is a DSPT requirement and provides evidence for CQC Well-led inspections.
Every significant action is recorded in the audit log:

| Login Activity | Successful logins, failed login attempts, MFA challenges |
| Patient Record Access | Viewing, editing, or creating patient records |
| Appointment Changes | Booking, rescheduling, cancelling appointments |
| Prescription Actions | Creating, signing, cancelling prescriptions |
| User Management | Creating users, changing roles, disabling accounts |
| Configuration Changes | Practice settings, opening hours, service changes |
| Compliance Actions | Creating/editing activities, risk register, IAR entries |
| Data Exports | Downloading reports, audit exports |
