Common issues and how to resolve them.
| Issue | Solution |
|---|---|
| Forgotten password | Click Forgot Password? on the login page. A reset link will be sent to your email. |
| Account locked | After 5 failed login attempts, your account is locked for 15 minutes. Wait and try again, or ask a Practice Manager to unlock it. |
| MFA code not working | Ensure your authenticator app's time is synced. Try generating a new code. If it still fails, use a recovery code (see MFA guide). |
| Page not loading | Check your internet connection. Try clearing your browser cache and cookies. Try a different browser. |
| "Invalid subdomain" error | Ensure you are using the correct URL: https://{slug}.veripath.co.uk |
If you have lost your phone and have no recovery codes:

| Page looks broken | Clear browser cache and hard reload (Ctrl+F5 or Cmd+Shift+R) |
| Buttons not responding | Disable browser extensions (especially ad-blockers) |
| "Cookies required" message | Enable cookies for this site in your browser settings |
| Slow performance | Close other tabs. Check your internet speed. |
