¶ Digital Patient Intake (Forms)
¶ What you'll be able to do
After reading this guide you will understand how patient details are collected before treatment through digital intake forms, and how the app tracks whether a patient's intake is done. You will be able to check the intake status on an appointment, see what the patient declared (allergies, medications, conditions), and complete the pre-treatment medical history check before any procedure goes ahead.
Who does this? The patient completes the intake form themselves as part of registering on the patient portal. Receptionists check that intake is complete on the appointment and carry out the pre-treatment medical history check before the patient is treated.
¶ How digital intake works
Before a patient's first treatment, the app collects their details through a digital intake form. This happens in stages:
- Account created — the patient registers on the patient portal.
- Profile completed — the patient adds their personal details.
- Intake completed — the patient fills in the clinical intake form (see below).
- Identity verified — the patient's identity is confirmed.
- Registration complete — the patient is fully registered.
The intake form asks for three things, recorded as standard clinical codes (SNOMED) so records stay consistent:

This information is stored securely — health details are encrypted at rest to meet DSPT (Data Security and Protection Toolkit) requirements. Never print or repeat a patient's clinical details outside the app.
¶ Step 1: Find the appointment's intake status
Every appointment has an intake form status, shown as a badge:

| Pending | The patient still needs to complete their intake form |
| Completed | The patient has submitted their intake form |
- Log in to VeriPath Dental.
- Click Appointments in the top navigation bar.
- Open the appointment for the patient you are checking.
If intake is still Pending, the patient has not finished their form — check with them before their visit so nothing delays treatment.
¶ Step 2: Open the pre-treatment check
Before any procedure, the clinical team must review what the patient declared on their intake form.
- On the appointment, open Medical History (Appointments → the appointment → Medical History).
- The page shows the Pre-Treatment Check banner — it must be completed before any procedure.
The check shows:
- Patient details — name, email, NHS number, and intake form status
- Symptoms — what the patient reported for this appointment, the appointment type, and the AI priority score
- Appointment details — date, provider, practice, and status
- Intake summary — the allergies, medications, and conditions the patient declared on their intake form
- Past appointments — the patient's recent visit history
¶ Step 3: Confirm the review
- Read through the patient's intake summary — allergies, medications, and conditions.
- Tick the confirmation box: "I confirm I have reviewed the patient's medical history, allergy information, and there are no contraindications for this treatment."
- Click Confirm Medical History Reviewed.
A confirmation message appears and the review is recorded against the appointment. If anything in the intake summary needs attention, tell the treating clinician before the appointment starts.
¶ Good practice reminders
- Check the intake status on every appointment you book — flag any that are still Pending.
- Complete the pre-treatment check before any procedure, never after.
- Health details are sensitive: only look at what you need, and log out when you step away from your desk.
